Eligibility
Different benefits have different eligibility criteria. Generally, you are eligible for CAP benefits if you are:
- An active full-time employee working 30 or more hours per week*
*if you are a variable hour employee, you are eligible for medical benefits based on your Measurement Period.
Your dependents are eligible and include:
- Your legal spouse (including domestic partners**)
- Your child(ren) up to age 26
- Your disabled child(ren) up to any age (if disabled prior to age 19)*
*Includes natural, stepchildren who live with you, legally adopted/or a child placed for adoption, or a child under your
legal guardianship.
** Certificate of Domestic Partner registration is required to be submitted to HR if enrolling a Domestic Partner
Open Enrollment is April 19th - 26th , 2024
This is your once-a-year opportunity to pick your plans or make changes to your existing coverage. The choices you make during Open Enrollment will become effective on May 1, 2024 and will remain in effect through April 30, 2025.
When you enroll you can:
- Change your current coverage, if needed
- Enroll in benefits for the first time, if newly hired or newly eligible for benefits
- Update your dependent information
- Update your beneficiary information
Company ID
In order to enroll in your benefits, you will need to log in to Paylocity to make your elections. To log in, you will need to enter your Company ID. Please see below for a list of Company IDs. If you are unsure of your Company ID, please contact HR for assistance at egarcia@capphysicians.com.

Qualifying Life Event
Certain life events provide you a 30-day window to elect benefits or change your covered dependents.
What is a Qualifying Life Event? If you experience any of the scenarios below, you may be eligible to update your benefit elections mid-year:
- Marriage, divorce, or legal separation
- Birth or adoption of a child
- Commencement or termination of adoption proceedings
- Change in child’s dependent status
- Death of a dependent
- Loss or gain of other health coverage for you and/or dependents
- Change in employment status
- Change in Medicaid/Medicare eligibility for you or a dependent
- Receipt of a Qualified Medical Child Support Order
Remember, dependent verification will be required.
